When is it useful?
This is usually the right first step if you or a loved one:
- Has had a fall or fears falling at home.
- Shows changes in memory, attention or decision making.
- Is coming home after an illness or hospital stay.
- Needs help from family and wants a clear plan.
What the assessment includes
I visit your home and observe how daily activities are carried out: getting up, bathing, dressing, preparing meals, moving around and managing medication. Seeing the real environment lets me identify risks and opportunities that are impossible to notice in an office.
We talk with you and your family about habits, interests and goals. I use validated functional and cognitive tools to document where things stand today and how they change over time.
